Register To Appletree’s Family Practice
You must have valid coverage from the Ontario Health Insurance Plan (OHIP) and reside 100km or less from an Appletree Medical Centre to register.
If you already have a family doctor, you may not roster with our group.
Your current family physician should provide you with the majority of your care. You cannot be rostered to different groups simultaneously in Ontario. If you registered to Health Care Connect, you will be removed from their list as you roster to our group.
If you do not meet one of those criteria, you can continue to use our walk-in services!

Questions about our family practice?
Am I eligible to enrol in Appletree's family practice?
- Patients ages 16 and under are required to have a parent or guardian enrol on their behalf;
- Enrollment is available to patients with OHIP coverage only;
- Patient must be an Ontario resident;
- Patient must reside within 100km or less of one of our clinics.
What will happen after I sign up?
Once you have completed the form, you can immediately see any Appletree Family Physician via Appletree Virtual Care or in-person at a clinic. The registration form may take up to 7 business days to process.
How do I know who my family doctor is?
Once your registration is completed, you will be rostered with our Family Health Group. You may see any doctor accepting patients within Appletree’s Family Health Group. Our doctors work as a team, and through our network, they have secure and private access to your chart for shared care. Therefore you are welcome to meet with any Appletree family physician and receive continuity of care.
