What is a Third Party Physical?

A Third Party Physical is a visit with the doctor at the request of another party (the “third party”) such as an employer, licensing body, government, school, or insurance provider. The purpose of this visit is to complete a form, generate a written report, or obtain a Certificate of Good Health that can be given to the requesting party.

Airline employees
Federal government employees

Note: Appletree Medical Centres do not provide drug or alcohol testing on site. If required, patients will be provided with a requisition and directed to a local blood laboratory.

Cost

The fee for this appointment varies, depending on the service provided:

Ministry of Transportation: $125
Physical Exam for Form Completion: $171

Additional fees may apply if services such as Hearing Tests and Pulmonary Function Tests are required to complete the form.

How Can I Access This Service?

In-Clinic

This service is available at select Appletree Medical Centres. Appointments are required.

What to Expect During Your Appointment

During a third party physical examination, the doctor will follow the forms required by the third party. This usually includes your medical history, current medications, past surgical history, and social history.

When you arrive for your appointment, the staff will review your form(s) and perform any basic testing and measurements that are required. The doctor will make notes on your general appearance and then check your vital signs: temperature, pulse, and blood pressure. Height and weight will be measured, and in some instances, vaccines will be updated. The doctor may then proceed through a range of possible examinations on various parts of your body, such as a hearing test, a pulmonary function test, a Snellen test and a physical exam, as required.

After the physician completes your forms, the staff will make a copy and provide you with the original. Please ensure that you carefully review all sections of your forms for completeness and accuracy prior to leaving the clinic.

After Your Appointment

We will provide you with online access to all of your medical results and reports through our secure and confidential Patient Portal, so that you may access them from anywhere, at any time.

If blood tests or diagnostic imaging are required, you will be provided with a requisition and a follow-up appointment in one week to review the test results and complete the additional forms.
