After choosing the clinic nearest you with the shortest wait, you’ll check-in quickly using our self-registration kiosk, and provide your medical history while you wait. This will allow you to spend more time with the doctor focusing on your medical concern.

Prepare For Your Visit
If you are an Ontario patient, you will be required to present your OHIP card at every visit. Non-OHIP patients are always welcome and our staff will assist you with the registration process.

Don’t forget to bring your valid OHIP card and your updated list of medications so that we may update your chart with your current prescriptions and dosages.

Visit our wait-timer and select which location you will visit. We offer multiple locations and options for consistent care.
Check-In At Kiosk
After entering the clinic, you will be asked to complete your check-in at one of our kiosks.
Complete a Survey
You will then receive a link to a private and secure medical questionnaire on your mobile device. Answers will provide the doctor with a complete medical history and reasons for your visit.
Start Your Appointment
Our medical assistant will welcome you and get you set-up for your visit with an Appletree physician.
Visit our Patient Portal
Follow-ups, appointments and referrals can all be managed through our Patient Portal. Come back to any clinic and meet with any Appletree doctors to access your medical history from previous visits.
Appletree Walk-In Services
When you are sick, you can see a doctor quickly at any of our Appletree Medical Centres. All of our locations offer walk-in hours where physicians will see you without an appointment. If your injuries are major or you are experiencing severe symptoms like shortness of breath, loss of consciousness, chest pain, profuse bleeding, please go to the nearest hospital emergency room.

Do you have medical insurance through work, school or government?
Appletree Medical Group accepts coverage from a list of insurance providers. Patients with accepted medical insurance will need to bring proof of insurance to their visit.
