General Questions
Test Results Turnaround Times
Patients will be contacted to book a follow-up visit to review test results with a doctor, if necessary. Test results are not available on the Patient Portal, only their status is. View the status of your test results on our Patient Portal.
Test results are never discussed over the phone. If you do not receive a call and still would like to review the test results with your Doctor, you may return for a walk-in visit or schedule a Virtual Care appointment at your convenience.
If the turnaround time has passed for your type of test result and you do not see it on your patient portal, please reach out to the provider of the test and ask them to refax the results to us to the following fax numbers:

I need to review my test results. What should I do?
Test results must be reviewed with a doctor in-person or via Appletree Virtual Care.
Test results cannot be discussed over the phone, or by email, or via chat.

You can book an appointment with your preferred physician two ways:
Appletree Patient Portal : Our patient portal will allow you to book an appointment with a previously seen physician. If you want to book an appointment with a doctor you have never seen before, please call us.
Appletree Patient Portal Register or Sign-In
Appointments By Telephone: Please contact us by phone to book an appointment. We CANNOT book appointment via email, chat or using the contact us form on the website, you MUST call us.
Test Results on Appletree Patient Portal
Test results for which doctors have contacted you to review are typically not available on patient portal until they have been reviewed by you together with a doctor.

Can any doctor refer to a specialist at Appletree?
Yes, any doctor can refer a patient to a specialist. Patients are welcome to see any of the Appletree doctors through Walk-In of Family Medicine to obtain a referral. Family doctors outside of the Appletree network are also able to refer patients to Appletree specialists.
How can I obtain a copy of my medical chart? Is there a fee to transfer or get a copy of my records
A Medical Release Form must be signed by the patient (if 16 years of age and older), by the parent/guardian (if the patient is 15 years of age and under) or by the power of attorney (person given full legal permission with a signed document from a lawyer to make decisions on behalf of the patient) to request medical chart information. You can visit the Patient Resources section of our website to print, complete and mail or fax the Medical Release Form. Complete details are included on the actual form. Note that there is a $45 fee for Chart Transfers.
Are you able to renew prescriptions via phone or fax?
We are unable to renew prescriptions via fax or phone. However, you can visit any of our clinics, and a doctor will be there to help you.
Can I speak to my doctor over the phone? Can I review my results over the phone?
Doctors are not able to come to the phone as they are seeing patients. Please come in to see the doctor in person (or book appointment for patient if applicable). Results can only be discussed with the doctor during a visit.
I live in the province of Québec or I am from another Canadian province, can Appletree care for me?
Appletree welcomes all patients regardless of their province or country of residence. If you do not have coverage under the Ontario Health Insurance Plan (OHIP), you will be required to pay for the services you require.

How can I sign up for Appletree's Patient Portal?
You may register to our Patient Portal online by creating a secure and private account.
The Patient Portal will allow eligible patients to better self-manage their care, book appointments for certain programs, services or providers, view test results and much more. The Patient Portal is secure and private and grants patients unlimited access to their own health care profiles. To access and use the Portal, you must be 18 years of age or older.
What's the difference between a Virtual Care account and a Patient Portal account?
Appletree Patient Portal
Appletree Patient Portal allows you to monitor the status of your test results and keep track of your health. Our portal is free, convenient and easy-to-use for patients wanting more engagement with their healthcare.
Appletree Virtual Care Account
Your Appletree Virtual Care account allows you to manage appointments, update your preferences, pharmacy information, payments and profile.
Family Practice
I want to register with a specific doctor at Appletree.
At Appletree, our patients are registered to our family health group and not to a specific doctor. Once you are registered, you can choose to book with the same doctor at each visit. If you require care and your preferred doctor is unavailable, you are welcome to walk-in to any of our clinics or book an appointment with any doctor practicing within our network (in-person and virtual care).

Does Appletree offer Tuberculosis (TB) testing?
Yes. Appletree offers a skin test called the Mantoux tuberculin skin test. This test is performed by injecting a small amount of fluid (called tuberculin) into the skin in the lower part of the arm. After completing this test, the patient must return within 48 to 72 hours to review the results and look for any reaction on the arm.
The Ontario Health Insurance Plan (OHIP) does not cover the cost of TB skin tests that are required for employment, travel or volunteer purposes. To find out the cost of this test, review our Uninsured Services Price List.
Medically necessary TB skin tests and documentation are covered under the Ontario Health Insurance Plan (OHIP).

Do you prescribe narcotics or addictive medications?
Appletree physicians cannot prescribe narcotics or addictive medications to patients that are not their own, or for whom they will not be personally following up. Physicians will not provide prescriptions for narcotics or addictive medications to patients of other physicians, even those within our group.

I require a "Fit To Fly" certificate. What should I do?
Do you require a “Fit to Fly” certificate?
The Fit To Fly certificate is required for certain travels.
There is a $69 fee for this service, and OHIP does not cover it.

Booking Virtual Care
Please answer the questions below to book your Virtual Care appointment:
- Do you have coverage under the Ontario Health Insurance Plan (OHIP)?*
Select Coverage Yes, I am covered by OHIP or No, I do not have OHIP coverage. - Where do you live?*
Select Your Region I live in the Ottawa Region, I live in Ontario (outside of Ottawa), or I live outside of Ontario.
Once you have chosen your coverage type and the region you reside in, you will be brought to the next page. There you will be able to choose a video appointment. On the next page, please choose if the appointment is for yourself or someone else.
Once this has been completed, it will bring you to the page to choose your appointment type:
On the left, you have your reason for visit; please scroll down to “Fit to Fly” as the appointment type.
On the right will be a list of available Doctors; click on a Doctor to see available schedules and appointments.
After your appointment, the Doctor will email our Virtual Care team your certificate. The team will contact you to collect payment for your certificate and will be emailed directly to you once payment has been confirmed.
I need a Ministry of Transport (MTO) medical form completed for my license.
Our physicians provide MTO and Drivers’ Medical Examinations to professional drivers at Appletree Medical Centres across the Greater Toronto and Ottawa regions.
Drivers can book their Ministry of Transportation examination online at any time via Appletree Patient Portal.
We serve all professional drivers and students who need an MTO examination, including:
- Logistics/Trucking companies
- Police
- Firefighters
- EMS and Ambulance
- School Bus Drivers
- TTC, OC Transpo, GO Transit, and other Public Transit Drivers
- Waste Removal Workers
- Airport Workers
- Construction, Mining, and Heavy Equipment Workers

I have a question or would like to check on the status of an Attending Physician Statement (APS) request sent by my lawyer/insurance company.
The turnaround time for APS is up to 90 days.
If you would like to inquire about the status of your APS:
Please contact us by phone or send us a message including the following information: patient’s first and last name, date of birth, health card # (including 2-letter verification code) and the best phone number to reach you at.
Your request will be sent to our APS team, and they will contact you with the status of your request.
If your APS request was sent LESS than 7 business days ago:
Please contact us after a week, as it may take up to 1 week to receive and process the request in our system.
Do physicians accept to sign forms and reports as guarantors and what are the fees?
There are a number of forms for which you might need a guarantor’s signature or signature from your physician. Ontario physicians are not permitted to charge for the completion of some of those reports and forms. However, due to the requirements relating to be a guarantor (must be a person who has known you personally for at least two years and knows you well enough to confirm that the information you have given in your application is true), it is left at the discretion of the physician as to whether or not they agree to complete these forms.
Uninsured reports and forms include:
- Application for Accessible Parking Permit
- Transit forms for the Disabled
- Permanent Resident Card Forms
- Request for Birth Certificate Forms
- Children’s Aid Society Forms (on behalf of a child)
- Canadian Passport Application
- Ministry of Health & Long-Term Care Forms (e.g.: Limited Use, Assistive Devices, etc.)
If you would like to discuss the completion of any of the documents listed above, book an appointment via our Patient Portal or visit us at one of our Appletree Medical Centres for a walk-in visit.
OHIP & 3rd-Party Coverage
I am not covered by OHIP, may I still come in and receive care?
Yes. Patients not covered by OHIP, private patients, residents of other provinces or other countries, visitors to Canada and newcomers are welcome and will need to pay the service rate for a visit. Click here for current uninsured services price list.
What other forms of coverage are accepted? Can you bill my insurance directly?
We cannot directly bill to private insurances/out of province health cards. Patients must pay and keep their receipt to submit for their reimbursement.
Appletree accepts the following insurance upon receiving proof of valid coverage:
- UHIP University Health Insurance Plan (requires a $15 co-pay from the patient; the rest is billed to UHIP)
- CHIP College Health Insurance Plan
- Guard Me (Acute care only, some exceptions apply)
- MorCare
- Intrepid 24/7
- The Children’s Aid Society (CAS Child can be seen with or without health card)
- WSIB Workplace Safety and Insurance Board
Patients covered under other insurances or third-party payers not listed above are responsible for the cost of their visit.
Can I see a doctor if I misplaced my OHIP card?
OHIP cards must be valid and present for registration at every visit. Please contact Service Ontario, and they will provide you with the proper documents that you may use until your OHIP cards arrive in the mail.
